This SAME form will be used for all YOUTH and ADULTS and is valid for 12 months from date of Doctor's signature. This will be the ONLY Medical form accepted by the BSA for any and all Scouting activities that requuire a Medical Form. This new PDF file will let you fill in much of the form On-Line and print later. CAUTION - If you inadvertently CHECK-MARK a square box - the form WILL NOT let you UN-CHECK that box. You will have to cancel that copy and start over again.
